As a result of a complaint, the Inquiries, Complaints and Reports Committee decided to caution Dr. Ravneet Kaur as follows: • Chart notes must be done in a timely manner with little delay between the appointment time and the documentation. • If there is a need to delay entering chart notes, the notes must be marked as a “late entry” with the date of the event as well as the date they are entered recorded. • Charting must be fulsome and accurate at the time that it is done. If there is some need to later add to the notes, those additions must clearly be marked as a late entry as well. • When submitting records to the College, no changes are to be made to the original patient record or chart notes. Patient charts and records must be submitted as is. If additional information or explanation is needed, that can be provided separately but not under the guise of being part of the original patient record. • Informed consent must be obtained from patients by providing them with a full explanation of all of their options, including the details, risks and expected benefits of each. • Before embarking on a treatment plan, dentists must ensure that both they and the patient have a common understanding with respect to what treatment is being rendered. This need to ensure clarity and common understanding is even more important and may require lengthier discussions when the treatment is invasive, expensive and/or protracted over a lengthy period of time, as was the case in this instance. • Dentists are responsible for ensuring that consent discussions are ongoing, particularly as treatment plans and/or goals evolve.
